Who would be qualified for service?

Any Colquitt County student who has a physical condition medically diagnosed by a physician, which restricts them to their home or a hospital.

The hospital/homebound referral form must be completed in its entirety and returned to the attendance office, the principal, or designee at the school attended. This includes the medical certification section of the form, which is completed by the student’s doctor. Once completed, Student Services Director Doug Howell will determine whether or not the student is eligible for hospital/homebound services. If the student is eligible, services may begin immediately.

Responsibilities of the Parent:

  1. A responsible adult, other than the teacher, must be in the home while the teacher is present.
  2. The parent should provide an appropriate area in the home for the student and teacher during instruction.
  3. The parent shall see that the student is ready to begin instruction when the teacher arrives, including having assigned work completed.
  4. The parent should notify the hospital/homebound teacher in advance if the student is unable to receive instruction.

Responsibilities of the Student:

  1. The student should plan several times during the day when work can be done without interruptions in order to complete assignments in a timely manner.
  2. The student should inform the homebound teacher of any books or supplies needed from the school.
  3. The student must complete all given assignments.
  4. The student should be attentive and cooperative during the instruction period.

***Please be aware that not all courses can be completed during a homebound period, therefore it is a possibility that some course hours will be lost due to homebound studies.
